Nominations open for Healthcare Hero Award

FALLBROOK — The Fallbrook Healthcare District is seeking nominees for the next Healthcare Hero Award.

This is the second year of the program. Any person who resides in Fallbrook, Bonsall, Rainbow or De Luz is invited to submit the name of an individual who they think is deserving of being named Healthcare Hero 2010.

The purpose of the program is to recognize two individuals who, by the quality of their service, demonstrate exceptional commitment to the health and wellbeing of the residents of this community.

The nominee does not have to live in this community but their work must be of direct benefit to those who live in Fallbrook, Bonsall, Rainbow or De Luz.

The beneficiaries of the service of the nominee can be men, women and/or children. Those who benefit from the service of the nominee can be an individual, a family or a group of people.

The important factor is that the service contributes to health and wellbeing and is outstanding in its demonstration on a continuous and/or an exceptional circumstance basis.

It could be a volunteer, a staff person in a health service office, a coworker, a neighbor, a spouse, a friend – someone outstanding in their dedication to their endeavor to make a contribution to and a difference in the health of others.

The award consists of a personalized plaque to be presented at the annual Community Collaborative Breakfast on January 23, recognition published in local media, inscription on the Healthcare Hero perpetuity plaque displayed in the Fallbrook Healthcare District’s administrative office and honorary participation in district healthcare events throughout the year.

There is no monetary award. Under the Local Health Care District Law of the State of California codes and regulation, public funds cannot be paid to an individual. This is an honorary recognition.

A nominee submission form, with information to assist in preparing it, will be e-mailed, faxed or mailed to those who request one by calling or e-mailing the Fallbrook Healthcare District administration office.

Submissions must include the name, address and telephone number of the nominee as well as of the submitter.

Every nominee form must be signed by the person(s) submitting. Incomplete or unsigned submissions will not be considered.

A person can submit multiple nominees but each nomination must be on a separate form. Submissions can bear one signature or multiple signatures.

Submissions can be mailed, faxed or brought to the district administration office. E-mails cannot be accepted as nomination documents.

Nominations must be received by the Fallbrook Healthcare District by 5 p.m. on Thursday, November 19.

A committee of five will review the nominations and present a recommendation to the Fallbrook Healthcare District Board of Directors for their final selection.

The committee members are people who work (volunteer and/or paid staff) in various health and wellbeing entities in the community.

Names of nominees will not be published. The board’s selection will remain unknown until the presentation of the awards at the Community Collaborative Breakfast.
