Home health agency recognized for exceptional performance

FALLBROOK – The Centers for Medicare & Medicaid Services (CMS) recently recognized Fallbrook Home Health Agency for exceptional performance during a two-year demonstration project that was conducted in seven states including Alabama, California, Connecticut, Georgia, Illinois, Massachusetts and Tennessee.

Fallbrook Home Health was compared with other agencies in our region on seven quality measures taken from the Home Health Outcome and Assessment Information Set (OASIS). OASIS contains data items that were developed for measuring patient outcomes for the purpose of performance improvement in home health care. OASIS data items address socio-demographic, environmental, support system, health status, functional status, and health service utilization characteristics of the patient. “We are very proud to have been ranked in the top 20% of high performing agencies,” said Pat Smorzewski, RN, agency administrator.

Smorzewski attributes the outstanding performance of her agency to the commitment to quality care by the agency’s experienced and dedicated clinicians. “Our low staff turnover increases our ability to consistently provide a customized plan of care to our community residents” Smorzewski said.
