Healthcare District grant applications due March 23

FALLBROOK – The Fallbrook Healthcare District (FHD), in fiscal year 2011-2012, will again direct a portion of its revenues back into the community through the funding and sponsorship of programs and services that impact the health and well-being of the residents of the district.

Budgetary considerations of the district will be a determining factor in the amount of funds available as well as the number and types of programs that can and will be funded. Through its community healthcare programs, the grant program being one of them, the district will continue its mission to identify, promote and support a broad range of healthcare related needs and concerns within the community.

There are seven phases in the grant program process, beginning with submission of a Request for Proposal (RFP).

The RFP should be a brief, but concise, description of the proposed program, its target population, its cost and should indicate support of their own organization’s mission as well as that of FHD. The program proposed must be health related (treatment, education, prevention) and must be delivered by non-profit agencies benefitting residents of the Fallbrook, Bonsall, Rainbow and DeLuz areas.

Information and form packets will be provided by email, regular mail or may be picked up at the district office. Packets will need to be returned by mail or dropped off. Deadline for submissions is Wednesday, March 23.

RFP’s will be reviewed by the directors of the board either accepted or declined in the board meeting of Wednesday, April 13.

If the RFP is accepted, an invitation to present a full grant application will be extended by the board. If the RFP is declined, the agency will be notified and no invitation to apply will be extended.
